VNSNY Hospice and Palliative Care

About VNSNY Hospice

For 35 years, VNSNY Hospice and Palliative Care has provided comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families. We are one of the oldest and largest hospice programs in the New York metropolitan area, and the only hospice provider serving all five boroughs of NYC. In 2017 alone, we helped over 6,085 patients get the care they deserve.

Our goal is to ensure your comfort, enhance your quality of life, preserve your dignity, and respect your choices.

We’re here for you, every step of the way.

How VNSNY Hospice Care Is Different

As an illness progresses, it can be difficult to make choices about care or know what to expect. VNSNY Hospice and Palliative Care can help you explore your options. We respect your right to choose which services are best, as well as when and where you receive care.

Under your direction, your care team will work with your caregivers and personal doctor to develop a care plan tailored to your needs and goals. A dedicated interdisciplinary team of experts works to reduce unnecessary hospitalizations by educating you and your family and caregivers, and to streamline coordination of medical treatment.

Your services may include:

  • Medical care, including all medication, supplies, and equipment necessary to manage your symptoms and relieve physical discomfort
  • Personal care
  • Guidance on what to expect as your condition changes
  • Counseling for emotional and spiritual support from the onset of service
  • Help with practical issues, such as advance care planning, medical decision-making, and preparing for the future.

VNSNY also offers support, education, and counseling to family and loved ones to help relieve stress and anxiety, as well as a thirteen-month grief support program.

You’ll have 24/7 access to care and services. We’re here to support you however and whenever you need, including our dedicated caregiver helpline and referral center.
