VNSNY’s home health care services may include:

  • Skilled nursing care, to monitor surgical incisions and other wounds, review medications, and ensure you and your caregivers understand post-discharge instructions
  • Rehabilitation, such as physical or occupational therapy, to help you maintain or regain strength, balance, and coordination
  • Support and assistance from a home health aide, such as with eating or preparing food, bathing and dressing, or other household and personal tasks, as you recover
