VNSNY offers an evidence-based Behavioral Health program designed for psychiatrically and/or functionally homebound patients with anxiety, depression, dementia, bipolar disorder, or schizophrenia. VNSNY Telehealth provides in-home telecommunications solutions that help improve outcomes and reduce hospitalizations by tracking vital health information. They are particularly beneficial for patients with chronic illnesses who take multiple medications, or who require extra monitoring between physician or nursing visits.
