Home Care Services

As the nation’s largest not-for-profit home- and community-based health care organization, the Visiting Nurse Service of New York offers an unsurpassed breadth and depth of clinical solutions.

The service area of our certified home health agency (CHHA) covers the entire eight-county New York City metro region. We provide home care to more New Yorkers, and more hours of home health aide employment, than any other agency. With a staff that collectively speaks over 50 languages, we can meet the needs of New York’s extremely diverse communities.

Skilled Nursing Services

VNSNY provides skilled nursing care that incorporates best practices based on established national guidelines, including the AHA and the VNAA. Our goal is to maximize patients’ physical, cognitive, social, and behavioral well-being to achieve a higher level of independence at home and in the community. Our care and services help reduce avoidable rehospitalizations and hospital length of stay, improve recovery, and assist with safe transitions across settings.

  • Coordination of care and assistance in adherence to doctors’ orders
  • Pain management and medication reconciliation
  • Instruction in self-management of disease

Rehabilitation Therapies (PT, OT, SLP)

VNSNY rehab therapists work with patients recovering from surgery, injury, or serious illness to help them regain everyday functioning to the greatest extent possible. Our team includes over 460 rehab therapists, including physical therapists, occupational therapists and speech-language pathologists. Over 86% of them are full-time staff. They combine state-of-the-art technologies and best-practice protocols with hands-on care to achieve measurable outcomes.

Behavioral Health and Dementia Management

VNSNY offers an evidence-based Behavioral Health program designed for psychiatrically and/or functionally homebound patients with anxiety, depression, dementia, bipolar disorder, or schizophrenia. The program uses evidence-based screening tools to provide a comprehensive assessment that guides the creation of an individualized treatment plan. Through an interdisciplinary team approach, patients receive:

  • Care coordination
  • Medication management
  • Supportive counseling
  • Community support
  • Diagnosis-specific psycho-education, using self-management tools to help improve adherence
