Initiating the Home Care Conversation

Once you have identified a patient eligible for home care and made a referral, it is important that the patient and family follow through on your plan of care. However, many patients are unaware of:

  • What professional home health care is, and what benefits it offers
  • Why it is being prescribed for them
  • The extent of the clinical care VNSNY can provide
  • The efficacy of home health care

You can support your patient’s recovery by explaining the role and importance of home health care in:

  • Supporting patients within the community and at home
  • Assisting with adherence to the plan of care
  • Enhancing recovery

Initiating the Hospice Conversation

While starting a conversation about hospice with patients and their families can be difficult, the sooner the discussion begins, the sooner patients can get the care they need. The following questions often help start productive conversation:

  • What is your understanding of your disease?
  • What are your expectations/goals for the future?
  • What is most important to you at this time?
  • What treatment/care options are you considering?
