Finding the right home health care can be overwhelming. After you call VNSNY, we’ll take care of everything, so you don’t have to worry. We meet with the patient (and, when necessary, the family) to determine the appropriate level of care, and we put together a team of health care professionals to meet the patient’s needs.

VNSNY’s home health care nurse will coordinate and oversee most of the care. She will work with the patient’s physician to ensure that the patient receives proper care at all times, and she’ll schedule the other health care professionals—rehabilitation therapists, social workers, home health aides, and personal care workers or home attendants.
