Our Elder Care program can help. A professional consultation and care management service, we will provide you with the information and guidance you need to make the right decisions in caring for an older family member. One option is our At Home...Where I Belong® program, which is especially beneficial for adult children in other parts of the country who have an aging family member living in the New York City, Nassau or Westchester Counties.
We can also provide you with:
- Assessments, care planning and care management;
- Affordable options in dealing with physical, social, financial and other issues associated with aging;
- Help with preparing applications to receive Medicaid benefits, when appropriate;
- Specific recommendations and a plan of care.
Elder Care services can:
- Work with you to develop a plan that addresses the needs of both you and the elder.
- Screen, arrange, and monitor in-home help and community-based services.
- Coordinate use of all benefits, including Medicare and Medicaid when appropriate.
- Plan and budget your resources wisely to meet short-term and long-term needs.
- Help evaluate and select appropriate housing options, including assisted living, nursing homes, and senior residences.
- Assist with virtually any need, from daily money management to home modifications.
- Act as a liaison to families who are at a distance, serving as a local contact and helping to keep the family informed.
- Look in on the older adult and act as your eyes and ears while you are away.
