Nurse On-Site

If you live in a building that has partnered with VNSNY to become a congregate site, you or your loved one can feel secure in knowing that an on-site registered nurse is available to deliver consistent care when needed.

Our on-site nurses provide: 

  • Home health care
  • Health screenings and assessments
  • Group health education
  • Instruction to individual residents and caregivers
  • Assistance in making plans for hospital discharge
  • Advocacy with your doctor or insurance companies to make sure you’re getting the care you need, when you need it

VNSNY nurses can address your health concerns, answer any questions you may have, and help you stay healthy. In addition, they provide instruction and in-service training to the staff at the residence, and will work with building personnel to make sure you and your neighbors have the health services you need.
