Strong Foundations is an innovative four-part course of treatment designed to reduce the risk of falls.
Assessment This at-home evaluation by a registered nurse and physical therapist measures your loved one’s degree of risk for the eight factors associated with falling.
Treatment Plan The personalized plan designed around your loved one’s specific falls risk includes exercises for balance, walking and strength training; environmental adaptation to reduce risk factors in the home and in daily activities; correcting foot and footwear problems; providing education and information; minimizing medications; and treating and managing vision impairment.

Progress Tracking A Progress Log helps everyone (doctors, family, etc.) track the patient’s progress and keeps everyone up-to-date.
Results During the final visit, we reassess the patient and provide him or her with an updated exercise plan to maintain his or her progress.
