Medical Care Coordination

What is coordinated care? At VNSNY, our staff works with you and your doctors to develop a Plan of Care designed to meet all of your individual needs. We find you the right combination of services, from rehabilitation therapy to nutrition counseling to household support, and make sure that you receive a care program designed for your unique situation.

As a VNSNY patient, you’ll be assigned a primary care coordinator who will manage your Plan of Care. Your coordinator will:

  • Review your needs and abilities on an ongoing basis
  • Keep your doctors and family caregivers informed about your condition
  • Schedule visits by our skilled professionals
  • Determine whether specialists are required
  • Evaluate your recovery 

VNSNY works with you to adjust your Plan of Care as necessary, so you continue to make progress and achieve the best possible health.
