What is coordinated care? At VNSNY, our staff works with you and your doctors to develop a Plan of Care designed to meet all of your individual needs. We find you the right combination of services, from rehabilitation therapy to nutrition counseling to household support, and make sure that you receive a care program designed for your unique situation.
As a VNSNY patient, you’ll be assigned a primary care coordinator who will manage your Plan of Care. Your coordinator will:
