When you choose Long Term Home Health Care from VNSNY, you’ll be visited by nurses, licensed therapists, home health aides and other professionals who are experts at caring for people with chronic illnesses or disabilities.

Your designated registered nurse will work with your doctor to develop a comprehensive plan of care specifically for your needs. Your customized plan may include such services as:

  • Skilled nursing care with a Registered Nurse;
  • Certified Home Health Aide for assistance with activities of daily living and companionship;
  • Physical, occupational and speech therapy;
  • Medical social work, for help sorting through the complex health care system (including applying for Medicaid, or researching and applying for other benefits and entitlements).
  • Nutrition guidance to help you understand the right food to eat;
  • Respite care for caregivers so those you care about can have a break;

Other Benefits

You may also receive services such as:

  • Housekeeping, or assistance with chores such as laundry and cooking;
  • Social day care to allow you to interact with others and even make new friends;
  • Transportation to get you to and from your appointments;
  • Pastoral care for spiritual guidance,
  • Assistance with moving or with repairs to ensure your home will meet your current needs.

In addition, as our patient, you will have access to care around the clock. VNSNY provides you with a personal emergency response system. If you fall or have an emergency, you simply press a button and our operators will speak to you through the box to get you the help you need. For non-emergencies, we have a 24-hour hotline staffed by registered nurses, so if you don’t feel well or have questions about your health or medications, a nurse is always available.

Our clinicians and home health aides speak multiple languages, so if you are more comfortable speaking a language other than English, just let us know.
