If you or a loved one has been diagnosed with HIV or AIDS, VNSNY can help you to manage your disease. As early as 1981, VNSNY was working to address the needs of AIDS patients. We expanded our Long Term Home Health Care Program in 2000 to include the special needs of people living with HIV/AIDS.
On any given day, we are the largest provider of HIV/AIDS services in New York City.
VNSNY’s Long Term Care Program for HIV/AIDS encompasses an array of coordinated services that are tailored to meet your and your family’s specific needs. Your registered nurse will work closely with your physician to ensure that you receive the level of care you need. Your nurse will oversee every aspect of your care, from scheduling appointments with other clinicians to keeping your doctor up to date.
Our program is designed to be flexible, so your services may change as your medical and social needs change. Among the services you may receive:
Our goal: To help you manage your disease, adjust your lifestyle to prevent complications, and to stay as healthy as possible and avoid hospitalization.
