Are You Eligible?

VNSNY’s Long Term Home Health Care Program is available to all adults 18 years or older who have a medical condition that requires ongoing skilled nursing, supervision or monitoring and want to remain at remain home.

To qualify for VNSNY’s Long Term Home Health Care Program, you must:

  • Have a chronic medical diagnosis that requires skilled nursing care and assistance for more than 90 days;
  • Be age 18 or older;
  • Live in Manhattan, Queens, Brooklyn, the Bronx, or Nassau county;
  • Have Medicaid or are Medicaid eligible and need help to apply;
  • Meet the admission criteria for VNSNY and New York State and be able to remain safely at home;
  • Have your doctor’s approval.

Getting Started with LTHHCP

When you call VNSNY to ask about long term home health care, a registered nurse will visit you in your home to assess your health for free. After the nurse and your doctor develop your plan of care and your eligibility has been approved, your services will begin.

LTHHCP is not an insurance program. There are no pre-authorizations for any services. Most importantly your doctor is in charge of your plan of care. There are no networks, so you can keep your own primary and specialty care physicians. Our program is fully funded by New York State. All of your care, including any medical equipment or supplies, is provided at no cost to you.
