Long Term Home Health Care Program

Do you have a chronic illness or diagnosis that requires ongoing medical care? VNSNY’s Long Term Home Health Care Program (LTHHCP) provides an ongoing comprehensive care plan to patients with chronic or complex health needs who wish to continue living safely and independently in the comfort of their own homes and near the family, friends and neighbors who mean the most to them.

While the official name, as of 1990, is the Long Term Home Health Care Program, it is commonly referred to as the "Lombardi Program," after New York State senator Tarky Lombardi. Senator Lombardi championed the health care bill that made the program possible in 1977.

It’s nice to know that, while you are sick or dealing with chronic illness, you have the option to receive health care services right in your own home. The Long Term Home Health Care Program provides you with a dedicated staff that will coordinate your care and provide the services you need to stay as healthy as possible, prevent complications, and even to avoid unnecessary hospitalization.

LTHHCP is available to all adults who are 18 years or older with a chronic illness or diagnosis. If you qualify, you’ll be assigned a registered nurse coordinator who will work with your doctor to develop a plan of care that meets your individual needs. Your nurse provides skilled nursing care and will oversee the other licensed professionals you may require, including:

  • Dedicated rehabilitation therapists for physical, occupational or speech therapy to help you improve strength and coordination and communicate effectively;
  • A medical social worker assigned to your case to help you navigate the health care system;
  • A licensed nutritionist who, in conjunction with your doctor, works with you on following a healthful and nutritious diet;
  • Your own home health aide to assist you with activities of daily living, like bathing, dressing, walking and eating, as well as to provide companionship.

Our medical professionals are experts at caring for people with diabetes, heart disease, hypertension, COPD, multiple sclerosis, HIV/AIDS and other chronic illnesses. We’ve been providing home health care to New Yorkers for more than a century.

With VNSNY’s Long Term Home Health Care Program, you will receive the highest quality home health care—and if you qualify, there is no cost to you. Our program is funded entirely by New York State.
