If you or a loved one has a chronic illness, disability and is now finding it hard to manage basic day-to-day activities, you may someday find yourself considering long term care in a nursing home facility. You should know that VNSNY offers two alternatives that would allow you or your loved one to live at home, close to family, and remain in the community.
