Welcome to the SPARK Care Program. SPARK was created to help you, the patient, and your caregivers manage your chronic illness. This management includes education about your illness and treatment options, treatment of pain and symptoms you may be having, and understanding the unique needs of you and your family.

Your SPARK team includes a Nurse Practitioner (NP) who will be the cornerstone of your care while on SPARK. Your team may also include a Registered Nurse, Social Worker, and/or Physician depending on your individual needs.

What is a Nurse Practitioner?

  • A registered nurse who has additional education (usually a masters degree) and training in the treatment of both common and complex medical illnesses
  • NP’s can:
    • Diagnose illnesses
    • Order, perform and interpret blood tests, x-rays and other diagnostic tests
    • Prescribe medicines and treatments
  • NP’s are expert healthcare providers that blend both nursing and medicine to treat not just an illness but the whole patient.
