Senior Care

One of the most difficult decisions an adult caregiver can face is to determine when to get involved in an elderly loved one’s care. This is especially true when that person is one of the people who once took care of you—mom or dad. Here is where VNSNY can help. We take a look at the senior’s condition and needs, and work with family caregivers and the patient's doctors to develop a plan that is best for you and your family, whether at home or in a facility.

Our skilled nurses and certified home health aides offer a wide range of senior care services. Our professionals can:

Provide overall health assessment and coordination:

  • Work with your doctors and other health care providers
  • Develop a "plan of care," from health care services to household support
  • Create a long-term care plan based on your needs and preferences
  • Help fill out applications to receive benefits and work with other insurance agencies

Provide skilled nursing care:

  • Manage care for age-related illnesses, such as Alzheimer’s disease
  • Administer respiratory therapies
  • Provide stroke rehabilitation
  • Monitor general health and help manage medications
  • Help order medical supplies and equipment

Provide support with daily living and household management:

  • Help with bathing and dressing
  • Shop for and prepare meals
  • Perform household chores such as laundry and light cleaning
  • Provide transportation to and from medical appointments
  • Help arrange for legal and financial assistance

 Provide end-of-life care:

  • Provide round-the-clock nursing at home or in a facility
  • Supplement nursing home care
  • Offer emotional support and counseling
  • Give hospice care

Our Senior Care program can be extremely reassuring to those who have an aging family member in the New York area, but live far away or are unable to meet the often complex needsof an elderly loved one.
