At the Visiting Nurse Service of New York, we do more than just provide skilled intervention when a complication arises due to an acute or chronic illness. We offer wound care management and a variety of disease management programs like diabetes management. We teach patients self-care techniques so that complications can be prevented whenever possible.
Our Centers of Excellence and disease management programs are nationally known for their self-care strategies that help control the impact of acute and chronic illnesses such as diabetes, heart failure, cancer, HIV/AIDS, asthma, as well as reduce the complications of wounds and ostomies (An ostomy is a surgical procedure in which a diseased part of the small or large intestine is removed and the remaining intestine is attached to an opening in the abdomen.)
Staffed by Clinical Nurse Specialists who are advanced-practice nurses with years of experience in their respective fields of nursing, our Centers of Excellence and Disease Management programs:
- Teach patients and their families self-care strategies that will help improve their quality of life though proper methods with things like wound care management and diabetes management
- Encourage and promote patient independence so that emergency room visits and costly hospitalizations can be avoided
- Develop nursing protocols and programs in the specialty areas of each program
- Provide ongoing education to nurses and other caregivers about the most current care standards
