The Visiting Nurse Service of New York provides care to seniors wherever they live, including in nursing facilities. If you have an older family member in a nursing facility or who needs nursing care, rehabilitation therapy or social work, our caregivers will provide the services needed, right in the nursing facility. We will work with you and the senior's doctor and nursing facility to provide the best possible care.
We offer a wide range of services to nursing facility patients, everything from skilled nursing care after an operation to geriatric care planning to rehabilitation therapy following a stroke. If you have a loved one whom you'd like to bring home or move to, say, an assisted living facility, we can provide an assessment in the nursing facility to determine if such a move is appropriate and, if it is, the scope of home health care services required. We can also make the necessary arrangements to ensure a smooth transition from the nursing facility to the home or other facility. If day-to-day care is needed so that the senior can remain at the facility or at home, we can provide those services, too.
