At the Diabetes Management Center of Excellence, our goal is to provide the highest standards of diabetes patient care and diabetes self care. To that end, we have developed diabetes care strategies focused on both patients and caregivers. Patient consults are provided by a VNSNY diabetes clinical nurse specialist who works closely with the nurse coordinator of care and contacts the patient's physician as needed.
What's more, our diabetes care clinical nurse specialist creates self-learning guides for our staff and patients and keeps nurses who provide the ongoing care abreast of the most current diabetes patient care standards. In addition to providing unsurpassed diabetes patient care, our nurses help both insulin-treated patients and non-insulin-treated patients learn how to live with their illness and become more capable in the self-care of their conditions.
Our nurses also teach diabetes self care to patients. This includes how to self-inject their insulin as prescribed, monitor their blood glucose levels, plan meals, and recognize the symptoms of potential complications. What's more, they have the expertise to use state-of-the-art medical equipment that assists with insulin administration.
