"My 86 year old mother lives in an apartment complex that has a high concentration of elderly residents. A few of them are very frail and, I think, need care on an ongoing basis, and all of them could benefit from health screenings and education. I've heard you have a program that places a nurse at such sites to provide these type of services. Could you please tell me more about this program."
Our Congregate Care program provides home health care services to elderly and disabled persons residing in congregate settings - adult homes, assisted-living facilities, senior citizen housing and naturally occurring retirement communities. A skilled nurse works on-site at the congregate care setting to deliver the care that the residents need to help them maintain their health and avoid hospitalization.
This program is available to people residing in:
- Assisted Living Facilities
- Adult Homes
- Public Housing for the elderly and disabled
- Section 202 supportive housing for the elderly
- Single room occupancy (SRO) facilities
- Naturally occurring retirement communities (NORCs)
- Senior housing
Our on-site nurses provide a wide range of services.
With our Congregate Care program, one or more registered nurses provide consistent care to residents who need it. The on-site nurse provides and coordinates the care of the individuals requiring certified home care services from VNSNY. Our Congregate Care program also provides on-site home health aides. To achieve more efficient personal care, we "cluster" our home health aides. Here's how it works: imagine a patient who requires assistance with personal care, meal preparation and assistance with eating. Since these are tasks that span the day, the home health aide makes multiple visits throughout the day. As a result, the patient receives the expert personal care he or she needs but in a more efficient, less costly manner.
Our on-site nurses provide:
- Certified home care, as needed
- Health screenings and assessments
- Group health education
- Instruction to individual residents and caregivers
- Assistance in making plans for hospital discharge
- Home health nursing consultation/liaison to management and residents
- In-service training to residence staff
How to find out if our program is right for you.
In most locations, there is no formal arrangement between VNSNY and the residence, but the volume of home health care cases results in VNSNY assigning one or two nurses to the residence. In some locations, an agreement is developed and a nurse is placed on-site with a designated space. Our program accepts Medicare, Medicaid and other types of insurance. The VNSNY Congregate Care Program is available to facilities located throughout the Bronx, Brooklyn, Queens, Manhattan, Staten Island as well as Nassau and Westchester counties.

Anyone can refer a patient - facility personnel, family, friends, neighbors, physicians, social workers. Simply give us a call. After a referral is made, a VNS nurse will evaluate the resident's need for home health care services. If care is needed, the resident's physician is contacted.
