Our At Home Where I Belong program is a breakthrough in home health care. This innovative program provides older adults the individualized care they need to grow old gracefully -- at home.
This program is for seniors who need or desire additional home health care services other than what Medicare covers but who do not qualify for Medicaid. We work with seniors and their families and doctors to create a total care program that meets all of the seniors’ health care needs. We can help with scheduling a trip to the doctor, assuring that an older adult is taking his or her medications properly, stocking the refrigerator with nutritious foods, and preparing meals. In short, we provide whatever type of care is needed. The program has all the benefits of an assisted-living residence -- but in the comfort and convenience of the senior's own home.
Our At Home Where I Belong program provides:
- Comprehensive, competitively-priced services to seniors and their families who are able to afford them privately
- A customized plan of care
- A "core" personal-care team that typically includes a nurse care manager, a supervisory RN, administrative supervisor and a home health aide
Here's how it works:
A registered nurse care manager visits with you to create a customized plan of care. The nurse care manager then manages and coordinates a personal-care team that's comprised of home health care professionals who will provide your care.
Depending on the person's individual needs, the personal care team may consist of:
- Nurse care manager
- Supervisory RN
- Home health aides
- Rehabilitation Services
- Physical Therapist
- Occupational Therapist
- Speech Pathologist
- Social Worker
We're responsible for your care -- all for one monthly fee.
We have the expertise and capabilities to provide you with a wide range of services:
- Care management
- Skilled nursing care
- Home health aides to help with such day-to-day activities as bathing, dressing, meal planning and preparation, laundry, shopping, and housekeeping
- Physical, occupational, and speech therapy
- Assistance with accessing entitlements and other benefits
- Money management
- Help with scheduling doctor and other appointments
- Transportation to and from doctors appointments (with an escort when necessary)
- Assurance that prescribed medication is being taken properly
- Arrangements for home health care medical equipment and supplies
