With over 9,860 highly skilled care providers, VNSNY is the largest not-for-profit home health care agency in the nation. In 2005, we made 2,200,000 professional home visits to more than 115,000 patients. From Flatbush to Riverdale, Chinatown to Harlem, New Hyde Park to Old Westbury, our caregivers travel throughout New York City, Nassau and Westchester Counties, seeing an average of 30,000 patients each day.

Licensed by the New York State Department of Health, we are fully accredited by the Community Health Accreditation Program (CHAP) of the National League for Nursing. Our CHAP accreditation - which also includes commendations for quality management and care coordination - assures you that the quality of care provided by VNSNY is nationally recognized for its excellence.

How Does VNSNY Provide Patient Services? The Visiting Nurse Service of New York (VNSNY) is a not-for-profit organization dedicated to home and community-based health care. The agency was established in 1893 by Lillian D. Wald, the founder of public health nursing in the United States.

Today, VNSNY is the parent organization of several not-for-profit companies whose staff provide and coordinate the care of patients residing throughout New York City and in Nassau County. The information below is designed to:

Explain the relationship among VNSNY's group of companies,

Describe the services available to patients and families through VNSNY's various companies,

Identify the services that may be provided, on a contractual basis, by companies other than VNSNY's own network of companies, and

Describe the payment methods for home health care

Patients who enter our care system may receive services from only one VNSNY company, or from several companies, depending on their needs. VNSNY staff will work with you and your physician to identify your care needs and either draw upon the resources available within our group of companies or inform you of private and other community services available.

What Makes VNSNY the Gold Standard in Home Health Care?

We have 110 years of home health care experience.

We have the capabilities and resources to provide the entire range of home health care services 24 hours a day, 7 days a week.

All of our caregivers are licensed professionals who we keep updated and trained in the latest care techniques.

We are fully accredited by the Community Health Accreditation Program (CHAP) of the National League for Nursing.

Our nurses are equipped with state-of-the-art hand-held, pen-based computers which they use in the home to retrieve a patient's complete clinical record, review and discuss medications and input new medical information about the patient.

Our Centers of Excellence and Disease Management Programs are leading the way in finding new ways to improve patient outcomes and the quality of life for patients with diabetes, heart disease, wounds and ostomies, asthma, HIV/AIDS and other illnesses.

Our nurses are the coordinators of all our home care services. The nurse assesses the patient's needs, identifies appropriate services and creates an individual plan. The nurse serves as the chief problem solver and communicates with other caregivers and the physician.

Our Center for Home Care Policy and Research conducts research that promotes the delivery of cost-effective care in the home and community and supports informed decision making by policy makers, payers, providers and consumers of home and community-based services.

Our RNs and Advanced Practice Nurses have an average of 13 years of nursing experience.

We offer specialized programs that address the language and cultural needs of New York's diverse communities.

Our Community Mental Health Services offer a wide range of programs that serve the severely and persistently mentally ill, provide mental health counseling, mobile crisis intervention, intensive case management, and services for the homeless.

Since our founding in 1893, the Visiting Nurse Service of New York has been a vital part of New York's health system and public health infrastructure, delivering care to New Yorkers of all ages and walks of life, including those who are underinsured or lack the ability to pay for our services. In 2005, we provided more than $23.3 million in charitable care and public health service to the New York community. Our charitable care dollars went to provide flu shots to the elderly, support and counseling to hospice patients and their families, as well as to fund unique community-based services for HIV patient and people with chronic mental illness.
