Long Term Home Health Care is for individuals who need help managing the on-going symptoms of chronic illnesses such diabetes, hypertension, COPD, multiple sclerosis and HIV/AIDS. If your loved one has a chronic illness or diagnosis, you might want to consider VNSNY’s Long Term Home Health Care Program (LTHHCP). Offering comprehensive medical care to those with complex needs, this program is designed to give your loved one the freedom to live safely and independently in the comfort of his or her own home.
Each member of LTHHCP will be assigned a registered nurse coordinator who can create an individualized plan of care that fits your loved one’s specific medical needs. Services include everything from skilled nursing care and rehabilitative therapy (physical, occupational and speech) to nutritional guidance and assistance with daily activities such as housekeeping, bathing, laundry and cooking. Additionally, LTHHCP provides medical social work services to help you sort through complex health insurance issues, as well as social day care services that allow your loved one to interact with others and make new friends. There is even a respite program to help relieve caregivers when they are unable to arrange care for their loved ones.
Another LTHHCP benefit is around-the-clock access to VNSNY’s personal emergency response system. In the case of an emergency, all one has to do is simply press a button and VNSNY operators will immediately respond to the caller through the box to get your loved one the help that he or she needs. For non-emergencies, VNSNY has a 24-hour hotline staffed by registered nurses.
LTHHCP is available to individuals who meet the following criteria:
LTHHCP is not an insurance program. There are no pre-authorizations for any services and there are no networks. This program is fully funded by New York State.
