VNSNY Long Term Home Health Care Program

Our Long Term Home Health Care Program (LTHHCP) gives patients a no-cost alternative to residential health care facilities. Since this is not managed care, there are no provider networks. Your patients have the freedom to keep you as their doctor and there is no impact to your billing reimbursement. 

Your patients may qualify for the benefits of this program if they require on-going home nursing due to a chronic medical condition, need assistance with personal care, are taking multiple medications or are frequently hospitalized. 

Patients must also be: 

  • Over the age of 18
  • Medicaid- or are Medicaid-eligible, and need help to apply
  • Residents of Brooklyn, the Bronx, Queens, Manhattan or Nassau County

All VNSNY nurses and home health aides are experts at caring for people with chronic illnesses such as HIV/AIDS, multiple sclerosis, COPD and diabetes. They’ll work with you to create individualized plans of care and help ensure patients follow your instructions.

Custom services may include:

  • Certified Home Health Aides
  • Physical, speech and occupational therapy
  • Medical Social Work
  • Nutritional guidance
  • Transportation and social day care
  • Full-service housekeeping
  • Medical equipment and supplies
  • Personal emergency response system (PERS)
