VNSNY offers evidence-based treatments aimed at improving the long-term health of at-risk patient populations. Our Transitions program was designed to reduce hospitalizations and uses predictive modeling to identify and determine appropriate care for patients who are most at risk. VNSNY Behavioral Health provides psychiatric nursing, cognitive therapy and specially-trained home health aides to help patients transition from in-patient settings back into the community. For elderly patients at risk for falling in the home, Strong Foundations combines skilled nursing with physical therapy to deliver treatment that has been proven to reduce falls and adverse consequences.

VNSNY’s Physician Web Portal enables you to quickly and securely manage your patients’ home health care online. With the Physician Web Portal you can:

8. HOME HEALTH CARE INNOVATION

Established in 1993 by VNSNY, the Center for Home Care Policy & Research is the only program of its kind in the nation. It conducts scientifically rigorous research to promote the delivery of high-quality, cost-effective care in the home and community. The Research Center:

• Uses VNSNY patient outcomes and translates the latest clinically proven practices into care
• Assists in developing and evaluating interventions and models to improve care
• Analyzes trends and models to impact future health care policy and research

VNSNY uses this research to improve health outcomes, develop specialized programs and enhance the quality of care for our patients. 

9. INSURANCE COORDINATION

VNSNY works directly with Medicare, Medicaid and private insurance plans in New York City. For patients who need additional assistance, our medical social workers can help them complete the necessary paperwork.
