Is your patient a candidate for Home Health Care?

Think your patient may need home health care? Use the Visiting Nurse Service of New York assessment checklist to help determine if he/she is a candidate.

Does your patient require one or more of the following assessments?

  • Wounds
  • Pain
  • Social surroundings
  • Environmental home safety
  • Self-care skills
  • Response to new treatments/medicines 

Does your patient require one or more of the following treatments?

  • Wound Care
  • Catheter/tube care (insertion, removal or maintenance)
  • Drainage tubes
  • Trach care (suctioning, oxygen)
  • Suture/staple removal
  • Infusion (antibiotics, pain management & nutrition)

Does your patient require one or more of the following education?

  • Medication administration
  • Disease management
  • Self care
  • Special diets
  • Assistive device, DME

Does your patient require one or more of the following rehabilitation therapies: Physical Therapy (PT), Occupational Therapy (OT), or Speech Language Pathology (SPT)?

Difficulty w/ ambulation (e.g. gait abnormality, new assistive device) PT
Difficulty w/ stairs PT
Fall risk or history of falls PT
Issues w/ bed mobility (e.g. scooting, turning, sitting up) PT and/or OT
Difficulty w/ transfers (e.g. toilet, tub, sit to stand) PT and/or OT
Weakness or deconditioning PT and/or OT
Patient lives alone – safety concerns  PT or OT
Dyspnea/difficulty breathing at any level PT or OT
Difficulty dressing/grooming OT
Difficulty bathing OT
Difficulty w/ feeding/meal preparation OT
Patient has help at home for self-care OT
Confusion at any level, including memory problems OT or SLP
Difficulty swallowing SLP
Difficulty speaking clearly SLP
