The Visiting Nurse Service of New York can be there to provide the home health care services your patient needs.
For your cardiac patient, VNSNY can:
- Monitor blood pressure and heart rate for medication adjustments
- Teach self-management techniques
- Assess the efficacy and side effects of all medications
- Provide rehab services
We’re available 24/7 to make sure your patient is following your plan of care and to keep you updated on your patient’s condition as often as you require.
Most patients can receive care in as little as 24 hours of your referral.
