The Visiting Nurse Service of New York is pleased to offer a ground-breaking diabetes management program, designed especially for teens. The services are aimed at teens and pre-teens with Type 1 diabetes and their families.
With the support of a VNSNY registered dietitian and social worker specializing in diabetes, the program guides teens toward making important behavioral changes. A Certified Diabetes Educator offers individualized education based on each teen’s condition and needs. The free program is coordinated through the child’s current physician, and each participant receives a cell phone equipped with a special diabetes application to help monitor and manage the disease.
